RDH Completion Application Instructions
Prior to beginning application, connect to a printer and have advisement sheet available. Application packet must include:
- Confirmation received upon submission of online application

Verification of current, unrestricted licensure as Registered Dental Hygienist from state authority; i.e. Georgia Department of Community Health
- Print and complete RDH Student Forms

Applicable Students - letter from previous RDH-BSDH completion program

International Students - copy of all immigration forms with visible expiration
Make a copy of all documentation for your personal records. Mail complete application package to: Clayton State University, College of Health - RDH Completion Application, 2000 Clayton State Boulevard, Morrow, GA 30260.
