Affidavits

When Clayton State University is the service provider party in a contract that requires the execution of an affidavit (pursuant to  O.C.G.A. 13-10-91 et. seq.), the authority to  sign the affidavit as officer or agent of the University has been delegated to Amy Shephard, Procurement Director. She may be contacted for execution of this contract affidavit.  A current and signed copy is available in the left margin of this page under "Clayton State University Affidavit".
