CSU Affidavit (when CSU is the Service Provider)

When Clayton State University is the service provider party in a contract that requires the execution of an affidavit (pursuant to  O.C.G.A. 13-10-91 et. seq.), the authority to sign the affidavit as officer or agent of the University has been delegated to the Procurement Director.  The Director be contacted for execution of this contract affidavit.
