Fitness Center Membership Forms

• Payroll Deduction (Faculty/Staff)
• Payroll Deduction Cancellation (Faculty/Staff)
• Alumni (Alumni Association)

Currently enrolled Clayton State University students have automatic membership through their student activities center fee.

Faculty & Staff are invited to join the Fitness Center for $20.00 a semester.

Alumni are invited to join the Fitness Center through the Alumni Association.

All patrons must present a valid Clayton State University Laker Identification card to use the facilities. These policies are in place for your safety and the safety of all the students/faculty and staff at Clayton State University. Each member must sign the required fitness center waivers before using the fitness center for the first time.
