For the 2021-2022 process, students who are chosen for verification will need to obtain their Required Action Letter from the DUCK Message Portal. All instructions must be followed on all Required Action Letters which may include completing, signing and returning the letter and submitting additional documentation to our office for processing.
IF REQUESTED, FORMS LISTED BELOW MUST BE SUBMITTED TO THE FINANCIAL AID OFFICE. Our office images all documents and routinely destroys all paper.
All documents submitted to the Financial Aid Office must be signed. Unsigned documents will delay the processing of your financial aid.
*Please allow a 24-hour turn-around period for confirmation of receipt. During peak times, please allow a 48-hour turn-around period.

Tax Return Transcript
Need a copy of your Tax Return Transcript to complete your financial aid file? You have two easy and convenient options for getting copies of your federal tax return information (tax return transcript). Instructions are available on the IRS website.
Tax Transcripts must be mailed to your home address not to Clayton State University.
Unusual Enrollment History Appeal Form
This form must be completed and submitted to the Financial Aid Office in order to be reviewed for HOPE eligibility.
The Family Educational Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education.
FERPA gives parents certain rights with respect to their children’s education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level.
It is the policy of Clayton State University to release information to a third party only after authorization from the student.
This form must be presented along with a copy of the student's valid photo ID either in person to the Financial Aid Office, by fax or by email to FinancialAid@clayton.edu. No information may be given to the parent about a student’s financial aid account without the student first completing this form. Please note that this form only covers authorization to release financial aid information.
