What is a family practice? Family Medicine is the branch of medicine that is concerned with the diagnosis and treatment of health problems in people of either sex at any age. Practitioners of Family Medicine are often referred to as primary care providers (PCP).

What is a nurse practitioner? Nurse practitioners are registered nurses who have earned a masters or doctoral degree in nursing. Our nurse practitioners are board certified in family medicine. This allows them to diagnose and treat patients of all ages.

What if I have an emergency or need to contact a doctor after hours? If you need immediate medical attention, please call 911 or proceed to an emergency room.

  • Clayton State’s Primary Health Clinic policy does not permit us to call in prescriptions for narcotics or pain medication after hours.
  • Generally, we will not call in antibiotic prescriptions without first seeing you for the problem.
  • Please follow your insurance policy's guidelines regarding after hours care. Failure to do so may leave you liable for any charges that are incurred.

What forms are available for download from the Clayton State Primary Health Clinic? For your convenience, you can download and fill out many forms before your appointment. Save time and print your required forms before your visit.

Does the Clayton State Primary Health Clinic perform sports physicals? Yes. Also called pre-participation physicals, sport physicals make sure an athlete can safely play. Sports physicals also help to keep immunizations current and allow you to discuss any health concerns you may have with our nurse practitioner. Please call our Scheduling Department at (678) 466-5590 for an appointment.

I need a referral to a specialist. How is that handled? Referrals are handled through our office. Please contact our office at (678) 466-5590 during our normal business hours.

Where are you located and what are your hours? Clayton State University’s Primary Health Clinic is a community-based health center housed inside Clayton State’s University Health Services (UHS) facility at Clayton Station.

Directions From 1-75 Exit 233 Turn left onto GA-54N/Jonesboro Rd. Continue on GA-54 N/Jonesboro Rd. approximately 1.3 miles and turn right onto North Lake Dr.  Your destination is on the right: 5809 North Lake Dr. inside Clayton Station, Bldg 1000.

Our hours of operation are Monday thru Friday from 9:30 a.m. until 3:30 p.m.

Does Clayton State Primary Health Clinic accept my insurance? Clayton State Primary Health Clinic is affiliated with a large number of major insurance carriers. Please call (678) 466-5590 with questions about your insurance provider.

What should I do if I need prescription refills? If you need a refill on your prescription, call your pharmacy and have them make a request. Please do not wait until you are out of your medicine. Allow 24 hours for your pharmacy to request the refill and for our office to respond.

When will I receive my test results? You will be given specific follow up instructions at the time of your procedure or tests.  Your health care provider will discuss test results at your follow up office visit.  Most follow up visits are within 7-10 days following procedures or tests.  Many times it takes that long to get your test results back from the laboratory.
