Veterans Basic Data Worksheet (VBDS): After you have applied to Clayton State University and applied for VA Educational Benefits, please complete this form.  If basic information has changed (contact information, degree program, and benefit type) then please complete an updated VBDS.  

After completed, please send the completed copy to the Veterans Affairs email


Electronic VA Form 22-1990: Follow the prompts to complete the electronic application for educational benefits under the following benefit programs:

  • Chapter 33: Post 9/11 GI Bill®
  • Chapter 30: Montgomery GI Bill®-Active
  • Chapter 1606: Montgomery GI Bill®-Selective Reserve

Average processing time is 30 days. 

After the Department of Veterans Affairs process the application, they will make a decision

  • If approved, VA will send the student a Certificate of Eligibility to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it
  • If denied, VA will send the student a denial letter to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it

Electronic VA Form 22-1995: Follow the prompts to complete the electronic request for changing schools or program of study for educational benefits under the following benefit programs:

  • Chapter 33: Post 9/11 GI Bill®
  • Chapter 30: Montgomery GI Bill®-Active
  • Chapter 1606: Montgomery GI Bill®-Selective Reserve

Please forward a copy of the confirmation to the Veterans Affairs email when you receive it

Electronic VA Form 22-5490: Follow the prompts to complete the electronic application for educational benefits under the following benefit programs:

  • Chapter 35: Dependents Education Assistance
  • FRY Scholarship

Average processing time is 30 days. 

After the Department of Veterans Affairs process the application, they will make a decision

  • If approved, VA will send the student a Certificate of Eligibility to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it
  • If denied, VA will send the student a denial letter to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it.

Electronic VA Form 22-5495: Follow the prompts to complete the electronic request for changing schools or program of study for educational benefits under the following benefit programs:

  • Chapter 35: Dependents Education Assistance
  • FRY Scholarship

Please forward a copy of the confirmation to the Veterans Affairs email when you receive it

Electronic VA Form 22-1990e: If the service member (spouse/parent) has transferred Chapter 33: Post 9/11 GI Bill benefits to you (the student), please follow the prompts to complete the Transfer of Entitlement (TOE). 

Average processing time is 30 days. 

After the Department of Veterans Affairs process the application, they will make a decision

  • If approved, VA will send the student a Certificate of Eligibility to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it
  • If denied, VA will send the student a denial letter to student via mail.  Please forward a copy to the Veterans Affairs email when you receive it

VA Authorization to Release Information 2021-2022: To facilitate authorization for Clayton State University to discuss Veterans Affairs Educational Benefits with persons other than student, please complete and forward the saved copy to the Veterans Affairs email

VA Advance Pay Request: Qualifying students that opt to use this option, please complete and forward the saved copy to the Veterans Affairs email.  

Qualifying Students:

  • Request is at least 45 days prior to the first fee payment deadline (Students must be registered for the upcoming term prior to request)
  • Student must be enrolled at least half-time (in courses that are required for the completion of your degree program)
  • Student must have more than a 30-day break between semesters, with no payment for the break.
  • Eligible and receiving at least one of the following benefit types:
    • Chapter 30: Montgomery GI Bill®-Active
    • Chapter 1606: Montgomery GI Bill®-Selective Reserve
    • Chapter 35: Dependents Education Assistance

VA Yellow Ribbon Request: Qualifying students, please complete and forward the saved copy to the Veterans Affairs email.

Qualifying Students:

  • 100% eligibility for Chapter 33: Post 9/11 GI Bill®
  • Charged out-of-state tuition rates because student does not meet the criteria for in-state residency or any out-of-state waivers (including military personnel waiver and recently separated military waiver)
  • Clayton State University is determined the HOME Institution; therefore, student is not a Transient Student at Clayton State University. 

YELLOW Ribbon Funds are exhausted for the 2021-2022 Academic Year
